About NOBI
Nurse On Board (NOBI) employs highly skilled Registered Nurse Care Managers who represent the interests of their patients and family members, are the primary contact in the circle of care, and are perfectly positioned to assess their patients’ changing needs through interprofessional collaboration. The company was founded by Susan Hagar in 2015, with a small group of highly experienced RNs, and a big vision. Today Nurse On Board has expanded to offer services across Ontario.
The project being funded through the Connie Clerici Nurse Entrepreneur Award will bring the expertise of experienced RN Care Managers to People with Parkinson’s Disease to facilitate timely problem-solving, collaboration with appropriate health professionals, and create solutions to critical gaps in community care.
Currently patients with Parkinson’s Disease (PwP) do not have access to an organized, well-developed, multidisciplinary program within the community, and therefore are not well supported with their complex needs. NOBI has identified the need for a Community Interprofessional Program for Patients with Parkinson’s. Very simply, there is no community program for PwP, and the Movement Disorder Clinics cannot organize or support care in the community.

Susan Hagar is the founder of Nurse On Board, a private healthcare company that connects highly experienced Registered Nurses with patients across Ontario who require advocacy, navigation, and care management. Susan and her colleagues bring clinical expertise to the home, liaise with healthcare professionals, facilitate homecare services, and more. In 2021, Susan’s team began strategizing on a Community Interprofessional Program for People with Parkinson’s. This unique option will be tailored to address the complex challenges these people encounter, optimizing their quality of life through education on self-management, multidisciplinary collaboration, advance planning, and timely access to specialized support in the community.
